Accident Summary:

The pilot heard a loud whine and crack. The aircraft yawed to the left and the rotor RPM decreased. He autorotated to a rice paddy and the main rotor blades struck the tail boom. Engine failure

Accident Summary:

 AIRCRAFT 63-8685 DEPARTED BEARCAT AS THE LEAD AIRCRAFT OF A FLIGHT AS A LIGHT FIRE TEAM ENROUTE TO VINH LONG, RVN. THE UNIT WAS SUPPORTING THE 7TH ARVN DIVISION AND WAS ENROUTE TO VINH LONG TO REFUEL BEFORE REPORTING TO THE OPERATIONAL AREA VICINITY XS 0661. AIRCRAFT 63-8685 DEPARTED BEARCAT TRAFFIC AND FLEW DIRECTLY TO THE RIVER VICINITY YS 0499 AND DESCENDED AT 85 TO 90 KNOTS TO APPROXIMATELY 10-15 FEET WHERE 2 UNSERVICEABLE ROCKETS WERE THROWN OVERBOARD. AIRCRAFT WAS FLYING ON AN APPROXIMATE HEADING OF 150 DEGREES OVER THE RIVER WHEN IT IMPACTED WITH THE WATER AND ALMOST IMMEDIATELY SANK IN 20-30 FEET OF WATER. TWO OF THE SURVIVING CREW MEMBERS INDICATED THAT THEY HAD DIFFICULTY ESCAPING FROM THE AIRCRAFT AS THEY FORGOT TO UNFASTEN THEIR SEAT BELTS, ONE CREW MEMBER COULD NOT REMEMBER HOW HE ESCAPED FROM THE AIRCRAFT. ONE CREW MEMBER WAS UNABLE TO ESCAPE.\\
